Announcing the Caregiver Career Pathways Program

The 1Heart Cares Foundation is proud to announce the launch of our Caregiver CareerPathways Program—a new initiative designed to address one of the most urgent needs in our communities today: access to quality care for seniors and the growing shortage of trained caregivers.
Rooted in our mission to improve the quality of life for seniors and families, this program creates meaningful opportunities for individuals to enter the caregiving profession while expanding access to compassionate, reliable care for those who need it most.

Why This Program Matters
Across our communities, families are facing increasing challenges in finding qualified caregivers for their aging loved ones. At the same time, many individuals are seeking stable, purpose-driven careers but lack access to the training and resources needed to get started.

The Caregiver Career Pathways Program bridges this gap—creating a direct pathway from training to employment while ensuring seniors receive the dignity, care, and support they deserve.

What the Program Offers

As part of this launch, the 1Heart Cares Foundation is actively partnering with senior centers, adult day care programs, and community organizations to provide free on-site seminars and training sessions. 
These partnerships allow us to bring critical education, resources, and opportunities directly into the communities we serve—making support more accessible for seniors and their families.

Announcing the Launch of Our Transitional Care Support Program
A New Initiative from 1Heart Cares Foundation
We are proud to announce the official launch of the Transitional Care Support Program on April 1, 2026  —a vital initiative designed to support seniors in Los Angeles County during one of the most vulnerable moments in their care journey: the transition from hospital to home.

​Bridging the Critical Gap After Hospital Discharge
Research shows that the first 48 to 72 hours after discharge are crucial. Without proper support at home, many seniors face a high risk of complications and hospital readmission.
The Transitional Care Support Program was created to address this gap by providing immediate, short-term in-home care for seniors who cannot afford it.
Our Commitment
In 2026, our goal is to:
  • Serve 100 seniors across Los Angeles County
  • Deliver approximately 5,000 hours of essential home care
  • Provide 48 to 72 hours of care per qualified patient immediately following hospital discharge
This initial support can make the difference between recovery and readmission—offering seniors a safer, more stable transition back home.

Eligible seniors will receive:
  • Compassionate, professional in-home care
  • Assistance with daily living activities
  • Monitoring during the critical post-discharge period
  • Support that promotes recovery, dignity, and independence
Care services are delivered through approved, licensed home care providers who meet the Foundation’s quality standards.
